Do you often start new projects before completing a project, and/or have multiple incomplete projects? (The project car, the home repair, the “hobbies”)

Do you lack motivation, procrastinate, or put off doing things, especially things that need to get done? (dread, it requires too much mental effort, filing for that extension on the taxes - again)

Do you have difficulty keeping track of, or remembering where things are? (I know it’s around here some where, I’ve got half a dozen sockets of the same size because I end up buying a new each time because I can never find the size I need when I need it)

Do you fidget, squirm, tap you foot, or constantly need something in your hand (pen clicking, can’t sit still in school desk)

Do you daydream, zone out, or get lost in your thoughts? (Need to re-read, or not listening during a conversation because you’re too busy paying attention to your own thoughts)

Do you get easily distracted, or side-tracked? (What was I doing? Did I just pass my exit? I have homework every night because my peers distract me from getting my work done in class)

Do you find it difficult to be patient, or wait your turn? (Just look how long that line is, It would be better to save my money to get the nicer version I prefer, but I really want it now - sold.)
